Outreach Group Travel Medical Insurance is designed for missionaries or church groups who:

* have a group of 5 or more that will be traveling together, and

* are traveling to a destination that is not the country of citizenship of any of the

traveling members, and that is not the United States.

** Note: Non-US groups coming to the US on mission need to utilize the Patriot

Group Travel Medical Insurance plan rather than Outreach Group Travel.

Obtaining coverage is as simple as:

* Each Group must have 5 or more Members.

* Fill out the Enrollment Form with information on Sponsoring Organization and

each Group Member. Note there are 10 spaces for names on each form. Use extra

forms if you have larger groups.

* Calculate your Premium and select desired payment mode.
