If you are traveling in a group of 5 or more, and everyone traveling is younger than 70, read below !
Your Team Leader should collect the following information on all people traveling:
1) First and Last Name
2) Date of Birth (mm//dd/year)
3) Social Security number OR passport number -- only need one.
4) Beneficiary (for the Life insurance portion of the policy). Typically would be spouse,
mother or father -- if left blank it will go to the legal next of kin.

We suggest if applying online, that you print the final invoice page for your group records. All ID cards and policy information will be sent via regular mail within 7 to 10 business days unless you specify it be sent via Express mail for an additional charge. With the paper application, you have the additional option of having an instant email confirmation sent to you rather than having it mailed and having to wait a week or so.
